The Italian club, along with Efes Pilsen and Partizan Belgrade - are all 4-5 in Group D heading into the final week.
Milano finished strong, outscoring the Spanish giants 21-8 over the last 10 minutes.
Italy international Marco Mordente had 23 points to lead the winners.
Unicaja Malaga scraped a 77-76 win at Cibona Zagreb to remain one game clear at the top of Group A.
Marcus Haislip had 14 points and Senegal international Boniface Ndong 13 for Unicaja.
Pini Gershon led Maccabi to a 96-83 home win over Olympiacos, the team that sacked him in the middle of last season.
Both sides are now 6-3 in Group A with Maccabi playing at Unicaja Malaga next week in their last game in the pool.
